Cindy "Sam" Tyeryar, 55, of Falling Waters, passed away on Wednesday, August 8, 2018 at Hospice of the Panhandle Inpatient Facility.

Born December 21, 1962, in Frederick, MD, the was the daughter of Ethel Rice Swanson and the late Joseph Smith.

She is survived by her husband Glenn Balcom; one son Jake; one daughter, Jessica; three grandchildren; one sister; and one brother. 

Services and interment will be private. Memorial contributions may be made in her name to Hospice of the Panhandle 330 Hospice Lane, Kearneysville, WV 25430.
